Capecitabine is a kind of chemotherapy drug used to treat different types of cancer, including colon, rectum, gastric, stomach, pancreatic, and breast cancer.

How does Capecitabine work?

Capecitabine is a type of chemotherapy known as anti-metabolite. When you consume this anticancer medication, it gets converted into 5-fluorouracil (chemical) inside the body.

Fluorouracil is the active form of the drug capecitabine. This chemical slows down the synthesis of genetic materials such as DNA and RNA in the cancer cells.

The chemical prevents cells from making the genetic material to grow and multiply.

Cancer cells grow much faster than healthy cells, so this chemical affects them and kills them effectively. Therefore, cancer cells cannot grow and spread faster.

How to use Capecitabine?

Below there is information regarding typical administration of this drug. Note that your doctor's recommendations may differ based on your condition and course of treatment.

Use capecitabine exactly as directed by your oncologist.
Your oncologist typically prescribes 2 daily doses of capecitabine at an interval of about 12 hours.
Each dose is intended to be taken no later than 30 minutes after a meal.
Swallow the tablets whole without crushing, chewing or splitting them unless otherwise directed.
Prior to treatment, patients generally need to be evaluated for the presence of DPD deficiency associated with specific DPYD genetic polymorphisms, unless the start of treatment cannot be delayed. Severe or life-threatening toxicity occurs due to DPD deficiency.
Doses of capecitabine are routinely accompanied with blood count, renal function, liver function, and toxicities from treatment monitoring.
Seek prompt medical assistance in case of severe diarrhea, ulcers in the mouth, fever, dehydration, pain in the chest, skin allergic reaction, or painful redness and swelling of hands or feet.
In case you missed a dose, do not make up the missed dose.

Indications/usage

Capecitabine is an oral nucleoside metabolic inhibitor chemotherapeutic agent. Present labeling authorizes its use in the management of various types of cancers such as stage III colon cancer, locally advanced rectal cancer, unresectable or metastatic colorectal cancer, advanced or metastatic breast cancer, unresectable or metastatic gastric, esophageal, or gastroesophageal junction cancer, and postoperative (adjuvant) therapy of pancreatic adenocarcinoma depending on the specific regimen of treatment.

Capecitabine is metabolized in vivo in a series of steps to 5-fluorouracil (5-FU). It interferes with the synthesis of DNA and process of RNA in cancer cells, slowing down or stopping growth of tumor cells.

The medicine may cause tiredness and dizziness. Do not drive or operate heavy machines or anything demanding concentration until you know how the drug affects you.

The drug can make you more likely to catch infections or may worsen the current infections. Stay away from people with an infection that may spread quickly, such as measles and flu.

When taking xeloda breast cancer treatment, stick to the treatment cycle properly and take the drug on time with other medications properly.

It is necessary to take the table twice a day regularly or as directed by the doctor. Wait for at least 30 minutes after the meal to consume the drug.

Never alter the dosage or stop the treatment suddenly because it may put your life at risk or, even worse, your cancer.

Drug Interactions

The significant drug interaction for capecitabine involves vitamin K antagonists such as warfarin and causes clinically significant elevation of INR and serious bleeding. The patients treated with the medications need more frequent coagulation monitoring and potential dose modification.

Allopurinol decreases the efficacy of capecitabine metabolites and is generally to be avoided during the treatment. Capecitabine increases leucovorin toxicity and concentrations of phenytoin requiring their monitoring. Additional caution is required with other substrates of CYP2C9 and nephrotoxic medicines.
