How does Cyclophosphamide work?

Before using medicine, patients must consider the working mechanism of medicine and make the right decision to use it.

On the other hand, you must pay attention to how long does cyclophosphamide stay in your system and how it controls the cancerous effect.

Alkylating agent performs by damaging genetic material like DNA and RNA of cancer cells.

The main aim of this medicine is to impede the growth and multiplication of cancer cells in the body. The agent works in different forms.

It attaches the alkyl group to the DNA base and fragment by repairing the enzyme to restore the alkylated base.

It avoids DNA synthesis and RNA transcription from damaged DNA.

DNA damages through cross-link formation that avoids DNA separate for transcription or synthesis.

Medicine induces mispairing of nucleotides that go ahead to mutation.

Patients gain complete protection against cancerous cell growth and multiplication when taking medicine.

Usual Dosage

Dosage varies depending on the condition, age, weight, organ function, and efficacy of treatment.

According to official prescription information, when Cyclophosphamide injection is used as the only antineoplastic agent in patients without hematologic deficiency, an initial intravenous course may be 40-50 mg/kg divided over 2-5 days. Other labeled intravenous regimens include 10-15 mg/kg every 7 to 10 days or 3-5 mg/kg twice weekly.

Nowadays in oncology practice, cyclophosphamide is often combined with other medicines in chemotherapy treatment programs, and so the actual dose and schedule of treatment may differ considerably from these general regimens. Dose adjustment or treatment delay may be required due to decreased blood cell counts, organ dysfunction, toxicity, and co-administration with other chemotherapeutic agents.

1000 mg is package strength, but not standard dose of single patient. Patient-specific dose should be calculated by treating oncology team.

Your doctor will determine the optimal dose and treatment regimen for you.

Drug Interactions

Cyclophosphamide is metabolized in the liver with participation of several cytochrome P450 enzymes including CYP2B6, CYP3A4, CYP2C9, CYP2C19, and others. Drugs strongly inducing or inhibiting these enzymes may affect activation or clearance of cyclophosphamide and potentially change its efficacy and toxicity.

Other myelosuppressive or cytotoxic drugs may increase risks of low blood cell counts and infections caused by cyclophosphamide. Drugs causing heart, kidney, liver, or bladder injury may increase overlapping toxicities and require additional monitoring.

As cyclophosphamide causes strong immunosuppression, vaccination schedule, especially with live vaccines, should be checked by treating oncology team before vaccination.

Discuss with your doctor and pharmacist all drugs, supplements, and herbal products you are taking prior to treatment.
